Please click the “Get Started” link- then complete and submit a short form. You will be contacted within 24 hours to schedule an appointment with our provider.

  • Prior to your visit, you will need to complete forms that will be sent through the patient portal. You will be invited to the patient portal once your appointment is scheduled. You will also need to provide current insurance information, and a driver’s license.
  • At your visit, your medical history will be reviewed as well as current symptoms that affect your mental health and well-being. If hormone replacement is recommended, this will be discussed and prescribed at your visit.
  • We do not prescribe controlled medications such as stimulants or Benzodiazepines. Our focus is on improving your mental health symptoms by returning your declining/depleted hormones to a more optimized state.
  • We ask that you bring any current laboratory results you may have had with your other healthcare providers and this will be reviewed at your visit. Reviewing a complete set of bloodwork gives us a better overall picture of your health, as physical and mental health go hand in hand. If additional labs are recommended, this will be ordered.
